Finger extensor variability in TMS parameters among chronic stroke patients
BACKGROUND: This study determined the reliability of topographic motor cortical maps and MEP characteristics in the extensor digitorum communis (EDC) evoked by single-pulse TMS among patients with chronic stroke. METHODS: Each of ten patients was studied on three occasions. Measures included location of the EDC hotspot and center of gravity (COG), threshold of activation and average amplitude of the hotspot, number of active sites, map volume, and recruitment curve (RC) slope. RESULTS: Consistent intrahemispheric measurements were obtained for the three TMS mapping sessions for all measured variables. No statistically significant difference was observed between hemispheres for the number of active sites, COG distance or the RC slope. The magnitude and range of COG movement between sessions were similar to those reported previously with this muscle in able-bodied individuals. The average COG movement over three sessions in both hemispheres was 0.90 cm. The average COG movement in the affected hemisphere was 1.13 (± 0.08) cm, and 0.68 (± 0.04) cm) for the less affected hemisphere. However, significant interhemispheric variability was seen for the average MEP amplitude, normalized map volume, and resting motor threshold. CONCLUSION: The physiologic variability in some TMS measurements of EDC suggest that interpretation of TMS mapping data derived from hemiparetic patients in the chronic stage following stroke should be undertaken cautiously. Irrespective of the muscle, potential causes of variability should be resolved to accurately assess the impact of pharmacological or physical interventions on cortical organization as measured by TMS among patients with stroke
Magnetic Fields Recorded by Chondrules Formed in Nebular Shocks
Recent laboratory efforts (Fu et al., 2014) have constrained the remanent
magnetizations of chondrules and the magnetic field strengths at which the
chondrules were exposed to as they cooled below their Curie points. An
outstanding question is whether the inferred paleofields represent the
background magnetic field of the solar nebula or were unique to the
chondrule-forming environment. We investigate the amplification of the magnetic
field above background values for two proposed chondrule formation mechanisms,
large-scale nebular shocks and planetary bow shocks. Behind large-scale shocks,
the magnetic field parallel to the shock front is amplified by factors , regardless of the magnetic diffusivity. Therefore, chondrules melted in
these shocks probably recorded an amplified magnetic field. Behind planetary
bow shocks, the field amplification is sensitive to the magnetic diffusivity.
We compute the gas properties behind a bow shock around a 3000 km-radius
planetary embryo, with and without atmospheres, using hydrodynamics models. We
calculate the ionization state of the hot, shocked gas, including thermionic
emission from dust, and thermal ionization of gas-phase potassium atoms, and
the magnetic diffusivity due to Ohmic dissipation and ambipolar diffusion. We
find that the diffusivity is sufficiently large that magnetic fields have
already relaxed to background values in the shock downstream where chondrules
acquire magnetizations, and that these locations are sufficiently far from the
planetary embryos that chondrules should not have recorded a significant
putative dynamo field generated on these bodies. We conclude that, if melted in
planetary bow shocks, chondrules probably recorded the background nebular

The emergence of 4-cycles in polynomial maps over the extended integers

Interestingly, cycles of length 4, called 4-cycles, emerge for polynomials in
under the appropriate
conditions. The problem of finding criteria under which 4-cycles emerge is
equivalent to determining how often a sum of four terms is zero, where the
terms are times a product of elements from the list of primes. We
investigate conditions on sets of primes under which 4-cycles emerge. We
characterize when 4-cycles emerge if the set has one or two primes, and
(assuming a generalization of the ABC conjecture) find conditions on sets of

Demographic determinants and effect of pre-operative angiotensin converting enzyme inhibitors and angiotensin receptor blockers on the occurrence of atrial fibrillation after CABG surgery
<p>Abstract</p> <p>Background</p> <p>Atrial fibrillation (AF) occurs in about 27% to 40% of post cardiac surgery patients. AF following coronary artery bypass graft surgery (CABG) is associated with a two-fold increase in morbidity and mortality. Various demographic risk factors and medications have been studied to predict the occurrence of this arrhythmia. The role of angiotensin related medications on the occurrence of AF in CABG patients is not determined.</p> <p>Methods</p> <p>Retrospective clinical and statistical analysis was made of all the patients who had undergone CABG surgery at Lehigh Valley Hospital during the years 2005 and 2006. Patients with chronic AF and those undergoing valvular surgery with CABG were excluded. Statistic analysis included chi-square test for categorical and student t-test for continuous variables.</p> <p>Results</p> <p>757 patients (560 males and 197 females) were studied. AF occurred in 19% of the patients. Age (70.5 vs. 65.1, p < 0.005. OR per year of age: 1.02, 95%CI: 1.018-1.023) and presence of hypertension (OR: 1.92, 95%CI: 1.086-3.140, p = 0.025) were significantly associated with occurrence of AF. Neither ARBs (OR: 0.78, 95%CI: 0.431-1.410, p = 0.41) nor ACE inhibitors (OR: 1.01, 95%CI: 0.753-1.608, p = 0.63) reduced the occurrence of post operative AF. Patients with post operative AF had a significantly longer hospital stay (9.5 +/- 5.4 days vs. 6.9 +/- 4.3 days, p = 0.001).</p> <p>Conclusions</p> <p>Advanced age and presence of hypertension were independent predictors of post-CABG AF. Patients with post operative AF had significantly longer hospital stay. Neither ARBs nor ACE inhibitors were associated with reduction of post-surgical AF. Further studies are needed to better delineate the role of angiotensin related medications on reduction of post-surgical AF.</p
Dark Energy and Right-Handed Neutrinos
We explore the possibility that a CP violating phase of the neutrino mass
matrix is promoted to a pseudo-Goldstone-boson field and is identified as the
quintessence field for Dark Energy. By requiring that the quintessence
potential be calculable from a Lagrangian, and that the extreme flatness of the
potential be stable under radiative corrections, we are led to an essentially
unique model. Lepton number is violated only by Majorana masses of light,
right-handed neutrinos, comparable to the Dirac masses that mix right- with
left-handed neutrinos. We outline the rich and constrained neutrino

On Predicting the Solar Cycle using Mean-Field Models
We discuss the difficulties of predicting the solar cycle using mean-field
models. Here we argue that these difficulties arise owing to the significant
modulation of the solar activity cycle, and that this modulation arises owing
to either stochastic or deterministic processes. We analyse the implications
for predictability in both of these situations by considering two separate
solar dynamo models. The first model represents a stochastically-perturbed flux
transport dynamo. Here even very weak stochastic perturbations can give rise to
significant modulation in the activity cycle. This modulation leads to a loss
of predictability. In the second model, we neglect stochastic effects and
assume that generation of magnetic field in the Sun can be described by a fully
deterministic nonlinear mean-field model -- this is a best case scenario for
prediction. We designate the output from this deterministic model (with
parameters chosen to produce chaotically modulated cycles) as a target
timeseries that subsequent deterministic mean-field models are required to
predict. Long-term prediction is impossible even if a model that is correct in
all details is utilised in the prediction. Furthermore, we show that even
short-term prediction is impossible if there is a small discrepancy in the
input parameters from the fiducial model. This is the case even if the
predicting model has been tuned to reproduce the output of previous cycles.
Given the inherent uncertainties in determining the transport coefficients and
nonlinear responses for mean-field models, we argue that this makes predicting

A polymorphism of a platelet glycoprotein receptor as an inherited risk factor for coronary thrombosis.
BACKGROUND: Platelet glycoprotein IIb/IIIa is a membrane receptor for fibrinogen and von Willebrand factor, and it has an important role in platelet aggregation. It is known to be involved in the pathogenesis of acute coronary syndromes. Previously, we found a high frequency of a particular polymorphism, PlA2, of the gene encoding glycoprotein IIIa in kindreds with a high prevalence of premature myocardial infarction.
METHODS: To investigate the relation between the PlA2 polymorphism and acute coronary syndromes, we conducted a case-control study of 71 case patients with myocardial infarction or unstable angina and 68 inpatient controls without known heart disease. The groups were matched for age, race, and sex. We used two methods to determine the PlA genotype: reverse dot blot hybridization and allele-specific restriction digestion.
RESULTS: The prevalence of PlA2 was 2.1 times higher among the case patients than among the controls (39.4 percent vs. 19.1 percent, P=0.01). In a subgroup of patients whose disease began before the age of 60 years, the prevalence of PlA2 was 50 percent, a value that was 3.6 times that among control subjects under 60 years of age (13.9 percent, P=0.002). Among subjects with the PlA2 polymorphism, the odds ratio for having a coronary event was 2.8 (95 percent confidence interval, 1.2 to 6.4). In the patients less than 60 years of age at the onset of disease, the odds ratio was 6.2 (95 percent confidence interval, 1.8 to 22.4).
CONCLUSIONS: We observed a strong association between the PlA2 polymorphism of the glycoprotein IIIa gene and acute coronary thrombosis, and this association was strongest in patients who had had coronary events before the age of 60 years

Subtle balance of tropoelastin molecular shape and flexibility regulates dynamics and hierarchical assembly
The assembly of the tropoelastin monomer into elastin is vital for conferring elasticity on blood vessels, skin, and lungs. Tropoelastin has dual needs for flexibility and structure in self-assembly. We explore the structure-dynamics-function interplay, consider the duality of molecular order and disorder, and identify equally significant functional contributions by local and global structures. To study these organizational stratifications, we perturb a key hinge region by expressing an exon that is universally spliced out in human tropoelastins. We find a herniated nanostructure with a displaced C terminus and explain by molecular modeling that flexible helices are replaced with substantial β sheets. We see atypical higher-order cross-linking and inefficient assembly into discontinuous, thick elastic fibers. We explain this dysfunction by correlating local and global structural effects with changes in the molecule’s assembly dynamics. This work has general implications for our understanding of elastomeric proteins, which balance disordered regions with defined structural modules at multiple scales for functional assembly.United States.
